These self-administered questions can be used to inquire about the frequency of an individual's consumption of beverages containing caffeine. Various time frames, ranging from once per month to six or more times per day, are arrayed on the response card for the respondent to use in assessing caffeine intake. The protocol also includes scoring procedures to convert an individual's responses to an estimate of caffeine intake in milligrams.
