This questionnaire assesses personal and family history among adults of glaucoma, macular degeneration, cataract, and diabetic retinopathy, including history of any associated laser or eye surgeries. Glaucoma, macular degeneration, cataract and diabetic retinopathy are the most common forms of blindness in the U.S. and throughout the world. All of these conditions have complex inheritance involving multiple genetic and/or environmental risk factors. Family history of these conditions, especially in first degree relatives, is a significant risk factor for disease.
