A questionnaire to determine the presence of diabetes among a study participant's first degree relatives. Positive history of type 1 or type 2 diabetes in a first-degree relative is a strong risk factor for the development of diabetes. Familial risk is a stronger for type 2 diabetes than type 1 diabetes.
