A complete review of past family and social history is a review of two or all three history areas - past history (illnesses, operations, injuries, treatments), family history (medical events in the patient's history, including hereditary diseases or those which place the patient at risk) and social history (age apropriate review of past and current activities). A review of all three is required for services that, by their nature, include a comprehensive assessment or reassessment of the patient. A review of two of the three is sufficient for other services.
