The list of questions in this panel were proposed by the National Coordinator for Health Information Technology (ONC) and Department of Health and Human Services (HHS) in the Federal Register, Vol 79, No 38. Feb. 26, 2014, Proposed Rules. The questions (except those related to limited English proficiency and assistance needed) were adapted from questions published by Health and Human Services (HHS) in 2011 under Section 4302 of the Affordable Care Act. They are used to characterize functional disability and identify assistance needed for a specified disability. Health care providers may use the information to better accommodate and respond to individual patient needs.
