The Birth defects registry report for hospital healthcare providers contains a cumulative record of a patient's encounters for diagnosis and/or treatment of birth defects to send to birth defects registries. For Birth defects event reporting for ambulatory healthcare providers, use the Birth defects Outpatient Registry report [LOINC: 75609-8]. Birth defect reporting may include congenital or structural malformation, a biochemical or genetic disease, or any information relevant to incidents of birth defects. This term was created for, but not limited in use to, the HL7 Clinical Document Architecture (CDA) Hospital Birth Defects Event Report Document.
