A case report reportability response document from public health is generated in response to a case report from a provider. The document contains information about the initial case report, the reportability for such conditions, which agencies have been sent the case report document, which specific public health agency is responsible for follow-up on each reportable condition, the reporting time frame, and any additional reporting action(s). The document may also contain links to external references and supplemental information requests, public health contact information for the primary responsible agency, and additional information of relevance to the possible reportable condition. This term was created for, but not limited in use to, the HL7 CDA® R2 Implementation Guide: Reportability Response, Release 1 - US Realm (May 2017).
